Which is most important to document about immunizations in the child’s health history?

Parent(s)/Guardian(s) are responsible for updating Ottawa Public Health (OPH) every time their child receives immunizations given by their health care provider. Providing immunization updates to OPH helps protect your child and the public in case there is ever a vaccine preventable disease outbreak in our community.  Doctors are not mandated to report your child's immunizations to OPH.

You can update OPH using either the online Immunization Connect Ontario (ICON) Tool or the CANImmunize App

Online (ICON) OR CANImmunize

For more information about ICON, the CANImmuize App and other ways to report your child's immunizations to OPH, visit our "Updating your child's record" section below.

You can also call Ottawa Public Health (OPH) to request a copy of your child's record. Please note that it will take a minimum of 5 business days to process your request. OPH cannot release immunization records by email.

OPH does not release immunization records to a parent/guardian of any individuals 16 years of age and older. An individual 16 years of age and older must call to request their own record or access ICON themself.

Note: Your child's immunization record at OPH only contains immunizations reported by yourself and immunizations administered by a public health nurse, for example, at a school clinic. It is possible that your child may have received additional immunizations, please submit any missing information to OPH to ensure your child's record is up to date.

If you are using the CANImmunize App, the information in your App may not be the same as in Ottawa Public Health's record. Synchronization between OPH and CANImmunize App is coming soon.
